Summer Camp Application Materials

Thank you for your interest in ISVI's Summer Camp - 2008.  The three camps run
simultaneously from June 15 through June 21, 2008 on the beautiful campus of the
Illinois School for the Visually Impaired.  These camps are being offered at
NO COST to you.

Please read and print the following information carefully as directions and explanations
are provided that are designed to assist you in obtaining the necessary application
materials and offer helpful information for your child to attend ISVI's Summer Camp-2008.

TRANSPORTATION TO AND FROM CAMP:
If you choose, ISVI will provide transportation for your young person to ISVI's Camp.
SPECIAL NOTE:  If you indicate on the application form (4TH PAGE) that you want
your youngster to be transported to camp by ISVI, then you will receive an evening
telephone call during the week before camp begins in order to confirm final
arrangements; pick-up points, times, etc.

If you will be bringing your child to camp yourself, please report to ISVI's "Main"
Administration Building on Sunday, June 15, between 3:00 and 5:00 p.m. where
check-in will be conducted.

It is YOUR responsibility to pick up your youngster when Camp ends on
Saturday, June 21
.  Please make special note of this date.  Also on June 21st, there
will be a closing program for parents starting at 11:00 a.m. in the Joshua Rhoads
Auditorium at ISVI. This program signifies the end of Camp and will be followed
by a light lunch in ISVI's Dining Hall for all campers and their families.

REMINDER--All Camp Application forms MUST be received at ISVI no later
than June 2, 2008
.  This year this deadline will be strictly followed.

FIRST:
If you are planning to print and mail in the Summer Camp Application Forms,
please follow this link to send an e-mail to ISVI letting them know you will be mailing in your application forms from the online printable forms.  So Camp planners can best
plan for your child's placement in the camp of her or his choice, please
include in this e-mail the following information:
1)  Your Camper's Name
2)  Camper's Age
3)  Which Camp your young person wishes to attend
      -- Young Explorers
      -- Fine Arts Appreciation
      -- Technology For Today
4)  The date you will be mailing in your application forms

Second:
DIRECTIONS:
1.  It is strongly suggested that you print this page of directions so you can be
-    sure you have printed copies of all required documents.
2.  Below are individual links to 14 different forms, documents and directions/maps.
3.  The first six forms are required for all campers while the last eight are
     provided either for your information or to assist you if you have never
     before traveled to Jacksonville or visited the ISVI campus.
4.  Click on each of the links below.  Print and complete all "required" forms.
5.  Print the additional documents for your information in preparing your child to
     attend Camp.
NOTE:  All required forms must be fully completedALL portions of the
4-page Application Form must be completed.  Partially-completed applications
will be cause for a camper to be refused acceptance to attend the Summer
Camp programs.

APPLICATION DEADLINE for ISVI to receive all application materials is
JUNE 2, 2008 and will NOT BE WAIVED.

Summer Camp - 2008 FORMS

1.  Summer Camp Application (4 pages) REQUIRED FOR ALL CAMPERS

2.  Summer Camp Immunization  History (1 page)  REQUIRED FOR ALL
NOTE:  If you have a "Certificate of Child Health Examination" form to mail in,
you will not need this form.  If you do not have the State examination form, then
you must submit this Summer Camp Immunization History form.  The State of
Illinois law requires a mandatory TB skin test within one (1) year prior to arriving
for camp as well as an up-to-date Tetanus shot and up-to-date immunizations.
If  ISVI's -"Immunization History Form" is used in place of providing a
"Certificate of Child Health Examination," the Immunization History form MUST
be completed and signed by a licensed physician, nurse or other medical
personnel.

3.  Computer Use Agreement (2 pages) REQUIRED FOR ALL CAMPERS

4.  Letter pertaining to ISVI's Internet Policy - This letter explains the
importance and details of ISVI's Computer Use Agreement--Item #3 above)

6.  Movie Permission Form (1 page) REQUIRED FOR ALL CAMPERS

7.  "ISVI Recreational Horseback Riding Release Form" (1 page) REQUIRED
FOR ALL CAMPERS

8Camper's Luggage Tag (REQUIRED ON ALL PIECES OF LUGGAGE)
Print as many as needed.  Attach this luggage tag on each piece of luggage
your child will bring to camp.

9.  Campers Personal Inventory List (1 page) (What to bring and NOT to
bring to camp) -- If you choose to print this form, you can use it to help your
camper pack for camp, but it is also suggested that this form be packed in
your camper's suitcase
so ISVI staff can make certain all your camper's
belongings get put back in their suitcase at the close of camp.

*Especially helpful for campers' families who have never been to ISVI.

CAMPERS WHO ARE ACCEPTED TO ATTEND SUMMER CAMP-2008
will receive a Letter of Acceptance no later than June 9, 2008. If you
